LOW BACK SURGERY: LUMBAR DISCECTOMY AND FUSION You have undergone low back surgery that may have included lumbar discectomy, with or without a spinal fusion and must realize that this has been a major operation, which may take a prolonged period of time to heal and rehabilitate. Often times, patients will feel better after surgery, particularly with respect to leg pain and particularly if they are in "good shape" going into the surgery. Other symptoms, including numbness or tingling, may also fade after surgery, but may also be discovered when the leg pain or radiating pain subsides! The usual time for healing of low back surgery is 6-12 months. During this time, your low back is in near-constant jeopardy. You will be given a brace to wear postoperatively and this brace should be worn for a minimum of 3 months for discectomy. If you have had a fusion, the brace should be worn for 6-9 months. You must avoid contact sports or other situations that may jeopardize your health and particularly your low back. Such activities include: lifting, bending, stooping, car accidents, and contact sports of any kind. When in the car, drive defensively, wear an appropriate restraint and your brace. The rehabilitation process can be a slow one. But, if you adhere to the prescribed program, you will optimize your result and do as well as you may. Just as in knee surgery, back surgery requires extensive rehabilitation of the muscles, ligaments and tendons of the low back. Your surgery, even with a small scar, goes deep to the spine. The spinal muscles have been temporarily moved in order to reach the spine and these take time to heal and strengthen. The rehabilitation process is your homework and needs to be done on a regular basis in order to get a good result. If you had a fusion surgery, you cannot smoke, use nicotine, or any anti-inflammatory medications, i.e., Motrin, Toradol, Advil, Naprosyn, etc., for 9 months. Activities from Surgery Day to Two Weeks After Surgery You are restricted to home and your neighborhood. During this time, you may walk around, go up and down stairs slowly and must avoid sitting at all times, except for bathroom necessities. Sitting puts a particular strain and spreads the lumbar muscles, putting pressure on the entire suture line. Excessive sitting will stretch the wound, causing pain and disruption of healing. You must avoid any situation that may cause a fall, such as slippery and wet surfaces. You may shower, but you may not tub bathe for two weeks after surgery. After showering, promptly "pat dry" the wound. A slip and fall during the critical six weeks after surgery may result in a recurrent disc herniation, tearing of tissues, breaking of instrumentation, or dislodgement of the bone grafts. The brace is to be worn at all times, except when sleeping or bathing!

Although it was not explicitly part of their charge, several of the breakout discussions considered and reported on their perceptions of the past and potential "value added" of the CMG program to the scientific enterprise as well as to collaboration and work force issues.
