Methods: Seventeen patients underwent surgery, including 9 undergoing implantation at the time of tumor recurrence and 8 with newly diagnosed gliomas. Grade 4 astrocytomas were present in all but 3 of the recurrent tumor group, who had grade 3 tumors. At the time of surgery, 1 cc of tumor was collected for tissue culture and in vitro testing against various chemotherapeutic agents. Cytotoxic agents included BCNU or carmustine ; , among others. In vitro response IVR ; was graded on a 0-5 scale. The study endpoint was patient survival after surgery and Gliadel implantation. Data were analyzed for correlation between survival and in vitro response to BCNU. Results: IVR to BCNU was 0 5 in patients, 1 5 in 5, 2 and 3 5 in Patients whose tumors had any IVR to BCNU survived a mean of 11.8 months, compared to 8.5 months for the nonresponders. Among the patients with newly diagnosed gliomas, survival was prolonged from 11 to 12 months in those with stronger IVR. This result was statistically significant p 0.01 ; . Conclusion: In glioma patients undergoing surgery and Gliadel implantation for newly diagnosed tumors, IVR was modestly predictive of clinical response. This data will be analyzed again with a larger patient group followed for a longer time. In vitro testing may play a role in customizing implanted chemotherapy with agents other than BCNU for patients with gliomas and glucosamine.
