We have selectively depleted host-reactive donor T cells from peripheral blood stem cell PBSC ; transplant allografts ex vivo using an anti-CD25 immunotoxin. We report a clinical trial to decrease graftversus-host disease GVHD ; in elderly patients receiving selectively depleted PBSC transplants from HLA-identical sibling donors. Sixteen patients median age, 65 years [range, 51-73 years] ; , with advanced hematologic malignancies underwent transplantation following reducedintensity conditioning with fludarabine.

The clinical introduction of GnRH antagonists has made feasible the development of new paradigms for ovarian stimulation in IVF Bouchard and Fauser, 2000 ; . GnRH antagonist action is characterized by an immediate suppression of pituitary gonadotrophin release and a rapid recovery of normal secretion of endogenous LH and FSH Hall, 1993 ; . The mid-cycle LH surge requires the secretion of native GnRH and can therefore also be prevented or inhibited by GnRH antagonist administration. Recent studies have shown that GnRH antagonist, either at single or multiple doses, is effective in suppressing a premature LH surge during ovarian stimulation for IVF Frydman et al., 1991; Diedrich et al., 1994; Olivennes et al., 1995; Albano et al., 1996 ; and its clinical efcacy is presently being conrmed in large multi-centre phase 3 clinical trials unpublished observations ; . This novel approach in IVF allows for the initiation of a normal menstrual cycle with normal early follicular phase recruitment of a cohort of follicles. When, in women of normal reproductive age, ~10 healthy early antral follicles are stimulated to ongoing growth by the endogenous inter-cycle rise in FSH, the aim of exogenously administered FSH may merely be to stimulate growth of these follicles up to the pre-ovulatory stage De Jong et al., 2000 ; . By making optimal use of endogenous FSH, the amount of FSH administered for an adequate response should be substantially less. Additional studies should establish the optimal starting day and dose of exogenous FSH required to prevent the decrease in serum FSH concentrations responsible for single dominant follicle selection under normal conditions. So far, studies with GnRH antagonists all involve the initiation of FSH administration on cycle days 2 or 3. However, maximum endogenous FSH concentrations are reached on median cycle day 6, and dominance is achieved on cycle day 7 in normo-ovulatory women van Santbrink et al., 1995; Schipper et al., 1998a ; . Indeed, preliminary observations suggest that multiple follicle development and IVF pregnancies can be achieved with daily doses as low as 100 IU rFSH starting on cycle day 5 De Jong et al., 2000 ; . This concept is currently under investigation in a larger, comparative clinical trial. With the use of GnRH antagonists during the late follicular phase, nal stages of oocyte meiotic maturation can also be induced by the administration of rLH, rFSH, native GnRH or GnRH agonist instead of HCG Olivennes et al., 1996 ; . These approaches are currently under investigation and may also reduce the risk of OHSS De Jong et al., 1998.

Just a glimmer of light from Pfizer on its earnings call next week and Nektar's new CEO in the months ahead could get NKTR moving again, in our view. Until then, we're all in the dark. Alkermes ALKS: Buy ; : From the Dark Side to the Bright Side, CEO Pops Highlights in Simplest Terms; Diverse Portfolio Driving Growth. Recent Vivitrol hang up not withstanding, we heard CEO Richard Pops' succinct presentation of Alkermes' product portfolio at the conference. Laying it down in simple terms, he conveyed the positive in non-grandiose format that seemed to have trickled into the audience's minds, if the stock movement since the presentation is any indication moved up some 9% on Friday ; . ALKS is also a derivative call from AMLN which presented two days before highlighting Byetta LAR product which showed high efficacy. Although the stock failed to move significantly post Lilly's agreement designating Alkermes as the sole AIR Insulin producer last Monday, we believe that the presentation the following Wednesday successfully highlighted several products in its portfolio. First, the market potential of Vivitrol, despite its slower than expected launch, and the AIR insulin product which is in Phase 3 as well as the collaborative effort on Byetta Exenatide ; LAR with Lilly and Amylin AMLN: Hold ; . Amylin Pharmaceuticals AMLN: Hold ; : Puts Investors' Minds at Ease with Positive Exenatide LAR Data. Amylin started our coverage universe with a bang at the JPM Conference with illuminating data regarding Byetta LAR which showed 15 week efficacy resulting in a drop of 2.1% in HbA1C levels while losing 8.3 lbs. By all accounts Byetta is well on its way to being a blockbuster drug; however, our concern for Amylin's lack of diverse product portfolio and threat from DPP4 inhibitors temper our optimism for the stock, believing at current levels it's fully priced. However, during the Q&A, CEO Ginger Graham adroitly countered questions regarding its partner Eli Lilly developing its own DPP4 product, likely assuaging many investors' fear of the DPP4 threat. However, in our note published recently detailing Rx trends for Januvia Merck's DPP4 Inhibitor ; , data indicate accelerating new Rx growth for Januvia while Byetta seemed to lessen slightly. We continue to believe this data could be indicative of the future Rx trends, where Byetta's growth could be pressured due to increased competition in the form of DPP4's and inhaled insulin. Other Mentionables: Barr Pharmaceuticals Sheds More Light Post Pliva Acquisition, Shire Pharmaceuticals SHPGY: Buy ; Partners with Abbott for its Fosrenol Product. Barr disclosed more information regarding current Pliva integration as well as its business outlook regarding the separate moving business parts across U.S. and in EU. Based on its statements and our estimates, the ability for Barr to enter into new areas of research in biologics and generic biopharmaceuticals due to the Pliva addition, we see the product pipeline as well as ANDA increasing to effectively place it as the third generic company in the world. More detailed information will be coming in its new fiscal year format starting on the Q1: 07 earnings call. As for Shire, CEO Matt Emmens gave more details to its previous guidance, as well as disclosing the Abbott partnership for its Fosrenol product. Short-term catalyst rich in our view, Shire's growth is also driven by multiple market launches as in the case of Daytrana. Synopsis The FDA has approved FosrenolTM lanthanum carbonate ; , a non-calcium, non-aluminum phosphate binder that reduces high phosphorus levels in end-stage renal disease ESRD ; According to Shire Pharmaceuticals, less than a third of patients are able to achieve serum phosphorus control despite dietary restriction and medication. Studies of Fosrenol have reported reductions in serum phosphorus one week after starting therapy, with the majority of patients reaching target levels within eight weeks. Maintenance of reduction has been observed for up to three years in patients treated with Fosrenol in longterm, open-label extension studies. Drop-outs due to adverse events have been reported in 14% of Fosrenoltreated patients. Bone biopsies taken from patients who had been treated with Fosrenol for up to 2 years, showed that patients maintained their bone health status throughout the course of treatment. Fosrenol is formulated as a chewable tablet and will be available in 250 and 500 mg strengths. The recommended initial daily dose is 750-1500 mg for adults and most patients will require a total daily dose between 1500 mg and 3000 mg to achieve serum phosphorus control. Regulatory authorities in Sweden granted a marketing authorisation for Fosrenol earlier this year.

UNIVERSITY OF ALBERTA Academic Appointments Dr. Kamaljit Kaur is our newest tenure-track member in Medicinal Chemistry. The appointment is effective April 2004. Kamaljit Kaur received her undergraduate education from India, and her PhD in bioorganic chemistry from Case Western Reserve University, Cleveland, Ohio. After a postdoctoral stint at Wesleyan University in Connecticut, she immigrated to Canada in 2001. At Wesleyan she worked with Rex Pratt on design of antibacterial agents targeted toward penicillin resistant bacteria. Prior to joining the Faculty of Pharmacy and Pharmaceutical Sciences, she did postdoctoral work on antimicrobial peptides bacteriocins ; with John Vederas in the Department of Chemistry, University of Alberta. The research interests of Dr. Kaur are in Bioorganic and Medicinal Chemistry with emphasis on inhibitor design, enzyme mechanism, and molecular modeling. She is also interested in understanding the mechanism of lipid modification in low-density lipoprotein so called bad cholesterol ; during plaque formation.
