DRUGS FOR RRMS Recombinant interferon beta-1b IFN- 1b ; Betaseron; Berlex Laboratories Inc, Wayne, NJ ; , recombinant interferon beta-1a IFN 1a ; Avonex; Biogen Inc, Cambridge, Mass ; , and glatiramer acetate Copaxone; Teva Pharmaceutical Industries Ltd, Petah Tikva, Israel ; have been approved by the US Food and Drug Administration for patients with RRMS Table 1 ; . These 3 drug therapies were tested in separate multicenter, placebocontrolled, double-masked clinical trials. Key elements of the studies leading to their regulatory approval are summarized in Table 2. Interferon beta-1b therapy was tested in 372 patients at a dosage of 8 million IU MIU ; 250 g ; or 1.6 MIU 50 g ; by subcutaneous injection every other day for up to 5 years, compared with placebo. The primary outcome measure was the drug therapy effect on the relapse rate. Treatment with the higher dosage reduced the relapse rate by 33%, increased the proportion of relapsefree patients from 16% to 31%, and reduced by 2-fold the number of patients.
ABSTRACT Alcohol dehydrogenases ADHs ; are enzymes capable of catalyzing the intercoversion of alcohols to their corresponding aldehydes or ketones by using different redox-mediating cofactors. They are valuable tools for the biocatalytic synthesis of various chiral compounds, which are potential building blocks or intermediates in the organic synthesis of therapeutics in the pharmaceutical industry. In particular, ADHs from marine hyperthermophilic microorganisms have attracted special attention as catalysts in several industrial processes over the last two decades, mainly due to some unique properties, including their relatively broad specificity towards various carbonyl substrates, their high thermal and chemical stability, as well as their suitability for cofactor regeneration. Therefore, this review article describes ADHs from marine hyperthermophiles, including their activity screening, cloning, heterologous production, and characterisation. On the Stage Manager's opening night, there are no costumes, no cues to call and usually no applause. Opening night for the Stage Manager is actually the first day of rehearsal. The mood that is set for this first rehearsal will determine the productivity, creativity and general atmosphere of every rehearsal which follows. The two most important words of advice for a SM about to begin rehearsal for any production are, "Be prepared." Of hydrogenated soy phosphatidylcholine, cholesterol, and distearoyl phosphatidylethalomine, which serves as an anchor for polyethyleneglycol DSPE-PEG2000; ref. 19 ; . Pharmacokinetically, the area under the doxorubicin concentration-time curve is increased at least 60-fold with Doxil compared with free doxorubicin. Doxorubicin is released effectively after liposomal extravasation due to the difference in kinetics between its diffusion out of the liposome and its accumulation rate within the tumor, yielding tumor peak concentrations occurring up to 48 hours postdelivery 19 ; . Hyperthermia augments intratumoral accumulation of liposomes over that achieved by passive accumulation. Local hyperthermia increases tumor perfusion and further opens the permeable tumor microvasculature, resulting in significantly increased liposomal delivery 5 ; and extravasation 8, 20 23 ; . Normal tissues are resistant to moderate thermal effects on vascular permeability temperature 44jC; refs. 21, 22 ; . It has been demonstrated that temperatures of 40jC to 42jC are sufficient to increase tumor vascular permeability, with maximal extravasation rates occurring at 42jC, without damage to the normal vasculature 20 ; . These relatively mild hyperthermic temperatures are achievable in the clinical setting. Hyperthermic enhancement of tumor liposomal accumulation is greatest with 100-nm liposomes, compared with liposomes in the 200 to 400 nm size range 21 ; . Doxil liposomes, with an average diameter of 90 nm, have the ideal size to maximize the effect achieved by combining them with hyperthermia 21 ; . Several studies have shown that hyperthermia enhances intratumoral drug uptake and cytotoxic effects compared with those of liposomal drug alone or hyperthermia given with free drug 21 24 ; . Intratumoral drug concentrations are one of the best predictors of treatment outcome 1 ; . Thus, a noninvasive technique that could accurately quantify the magnitude of drug within an individual tumor, both with and without hyperthermia, is highly desirable and would find broad clinical application. Estimation of the expected intratumoral drug concentration might permit the selection of patients who are most likely to benefit from the combination of hyperthermia and drug-containing liposomes. Scintigraphic imaging is a valuable tool for this purpose. It provides the ability to track and quantify the distribution of liposomes in the body.
With a single i.v. injection of polymer on day 2, 4, 8, or 16 after tumor implantation. In the experiments intended to determine the effect of dosage on efficacy, 8 days after tumor implantation, mice were administered by tail vein injection a single dose of hydrazone-linked bow-tie DOX conjugate in 200 l of PBS at concentrations of 1, 3, 6, and 20 mg kg DOX. Control groups were treated with Doxil 6 and 20 mg kg DOX in 200 l of PBS ; , DOX HCl 6 mg kg in 200 l of PBS ; , and PBS 200 l ; . DOX HCl and Doxil were purchased from the UCSF hospital pharmacy. In the experiment intended to determine the antitumor effect of the hydrazone-modified bow tie lacking DOX and the bow-tie dendrimer with DOX attached by means of a stable carbamate linkage, animals were treated on day 8 after tumor implantation with either the hydrazone-linked bow-tie acetone conjugate 238 mg of polymer kg ; or the carbamate-linked bow-tie DOX conjugate 20 mg kg DOX ; in 200 l of PBS by tail vein injection. Control mice received 200 l of PBS and echinacea.

In cortex, Gb4 was expressed on tubules surrounding glomeruli and in the cortical labyrinths and the medullary rays. In medulla, Gb4 was expressed on renal tubules extending toward renal papillae. The expression pattern of Gb4 was compared with that of ACE and THP using serial sections Fig. 9 ; . As the expression pattern of Gb4 coincided with that of ACE, Gb4 appeared to be expressed mainly in the proximal convoluted tubule. Gb4 was stained more densely in the cortex than in the medulla in kidney, when compared to the staining of Gb3 CD77, which was stained almost equally between two regions. In lung, Gb4 was expressed on cilia of the ciliated pseudostratified columnar and doxil.

The ultimate goal is to show that this combination of drugs is more efficacious than vad or velcade with either dexamethasone or doxil and without additional toxicity and eletriptan.
