PREVENTION OF CONTRAST NEPHROPATHY CNP ; WITH HYDRATATION AND FUROSEMIDE FS ; A Eren, F Keller, P Jehle, R Lukas Division of Nephrology, Medical Department, University Hospital, Ulm, Germany The risk for CNP is increased for patients with renal insufficiency. Hydratation before and after contrast agents reduces this risk. We supposed that additional FS is favourable. In a randomised controlled study 30 patients, with serum creatinine 130-500 mol l and indication for radiocontrast investigation, received either hydratation group A ; or hydratation and FS group B ; . FS was given after the contrast media. The FS dose was 40 mg or twice the individual chronic diuretic therapy. The hydratation was made conform to the patients individual fluid balance. Creatinine change as measured in group A was from 252132 medianSD ; to 272234, in group B from 16363 to 15694. Five of 11 patients in group A and 2 of 19 patients in group B had a significant creatinine increase i.e. an increase of 44 mol l or more comparing the creatinine before contrast media and that 14 days after ; 2test, p 0.03 ; . One patient in group A needed dialysis after contrast media. Conclusion: In patients with renal insufficincy receveing contrast media FS, after and not before investigation, combined with sufficient hydratation, might be favourable.

| With all the value that may adhere to the true, the genuine, the selfless, it could be possible that a higher and more fundamental value for all life might have to be ascribed to appearance, to the will to deception, to selfishness and to appetite. It might even be possible that what constitutes the value of those good and honoured things resides precisely in their being artfully related, knotted and crocheted to these wicked, apparently antithetical things, perhaps even in their being essentially identical to them. Perhaps! -- But who is willing to concern himself with such dangerous perhapses! [Nietzsche, 1973, p.34] and apri.
